You can download the file, drag the PDF to open in google chrome, then print/save as PDF.
This will remove the password on any of the protected PDF

Impossible to open with google chrome. The rar need the password not the pdf file.

You don't need a password if you do the following:
1. Download the file
2. Extract the file with .rar converter (I used 'RAR extractor free' for MAC found in the app store). The program asked for the password and when I entered an invalid password, the program still extracted the file, but it was still locked.
Next, do one of the following (3a or 3b):
3a. Open with adobe reader and you can read it without a password.
or
3b. Drag the file into either a google chrome or safari browser. It will open the file for viewing.
4. Save as pdf. Then you will have a non-password-protected file!

change name of file using windows CMD as follows:
ren Clinical_Anesthesia_2C_7th_Ed.pdf Clinical_Anesthesia_2C_7th_Ed.rar
then extract the file
then again:
ren Clinical_Anesthesia_2C_7th_Ed Clinical_Anesthesia_2C_7th_Ed.pdf
